EDISC Diploma application

Dear EDISC course participants, if you have completed all your EDISC modules (modules 1-7), please apply below to receive your EDISC Diploma.

The processing of your diploma application may take up to 8 weeks. We thank you for your patience!

Are you eligible to apply for the EUROSPINE EDISC Diploma?

Participants in EUROSPINE EDISC Courses who have completed all modules are eligible to apply for the EUROSPINE EDISC Diploma.

EDISC modules
  • EDISC Module 1: Using literature to inform my practice (core skills 1)
  • EDISC Module 2: An interprofessional approach to spine care (core skills 2)
  • EDISC Module 3: Communication and consultation skills (core skills 3)
  • EDISC Module 4: Assessment, diagnosis and management of spinal pain
  • EDISC Module 5: The aging patient seeking spine care
  • EDISC Module 6: Spinal deformities - Scoliosis during growth & other deformities across the life course
  • EDISC Module 7: Management and prevention of disability

In order that we may check your eligibility to receive your diploma, please provide us with the CME certificates for all modules that you have taken (either face-to-face or virtual live). Only applications including all 7 CME certificates will processed!
  • Application for EDISC Diploma (EUROSPINE Diploma in Interprofessional Spine Care)

  • Please only use English alphabet/characters as we cannot process special characters. This is especially important for addresses as we may be missing the letters with special characters, which could jeopardise the diploma postal delivery.

  • I confirm that the above filled in names shall appear accordingly on the diploma.

    Only applications where all necessary CME certificates proving eligibility will be processed!

    Applicants are requested to provide their CME certificates proving attendance to all 7 EDISC Modules:

    • EDISC Module 1: Using literature to inform my practice
    • EDISC Module 2: An interprofessional approach to spine care
    • EDISC Module 3: Communication and consultation skills
    • EDISC Module 4: Assessment, diagnosis and management of spinal pain
    • EDISC Module 5: The aging patient seeking spine care
    • EDISC Module 6: Spinal deformities - Scoliosis during growth & other deformities across the life course
    • EDISC Module 7: Management and prevention of disability
