Dear EduWeek participants, if you have completed all your basic modules with EUROSPINE, please apply below to receive your Basic Diploma. The processing of your diploma application may take up to 8 weeks. We thank you for your patience!

Are you eligible to apply for the EUROSPINE Basic Diploma?

Participants in EUROSPINE EduWeek who have completed all mandatory modules are eligible to apply for the EUROSPINE Basic Diploma.

Mandatory modules
  • Modules 1-5 prior to 2020
  • Modules 1-6 if you have started taking your modules as of 2020. Module 6: Using literature to inform my practice has become mandatory as of 2020.

In order that we may check your eligibility to receive your diploma, please provide us with the CME certificates for all modules that you have taken with us.

2019 Hybrid EduWeek, 2021 Virtual EduWeek and future EduWeeks
Participants in 2019 Hybrid EduWeek (November 2019), 2021 Virtual EduWeek (June 2021) and future EduWeeks must also provide, in addition to the CME certificate of the "live" sessions, their CME certificates for the eLearning component of their modules. Completing the eLearning component of a module is a mandatory requirement in order to fully complete/validate the module and obtain the Basic Diploma.

Diploma Equivalence with other Societies

If you completed some of your modules with another Society as per our diploma equivalence programme, please apply to the Diploma Equivalence Multiple Societies application below.

Please ensure you also provide the CME certificates for the eLearning and Live components of the modules (for modules taken in November 2019 and June 2021 only).

This will ensure we can process your request in a timely fashion.
  • Application for Basic Diploma (EUROSPINE EduWeek participants)

  • Please only use English alphabet/characters as we cannot process special characters. This is especially important for addresses as we may be missing the letters with special characters, which could jeopardise the diploma postal delivery.

  • I confirm that the above filled in names shall appear accordingly on the diploma.

  • IMORTANT NOTE, PLEASE READ BEFORE APPLYING!
    Only applications where all necessary CME certificates proving eligibility will be processed!

    2019 Hybrid EduWeek (November 2019) and 2021 EduWeek participants (June 2021) MUST provide (MANDATORY REQUIREMENT!):
    • CME Certificates proving completion of the eLearning components of the module
    • CME Certificates proving completion of the "live" (face-to-face for 2019 and online for 2021) component of the module

    Participants who have completed some of their modules with other Societies as per the diploma equivalence agreement must apply through the "Diploma equivalence multiple Societies" application www.eurospine.org/multiple-source-equivalence.htm).

  • Upload here the CME Certificate for the modules you have taken "live" (either face-to-face or online live):

  • Upload below your CME certificates for the eLearning component of your module(s). This is necessary for all modules taken as of Hybrid EduWeek November 2019, EduWeek 2021 and any future EduWeeks:

  • Upload below your CME certificate(s) for EUROSPINE "Module 6: Using Literature to inform my practice" if you started taking your modules as of 2020. This module has become mandatory as of 2020.
